Tattoo Aftercare Guide — Day by Day Through Weeks 1 to 4

Complete tattoo aftercare guide from Zürich. Day by day through weeks 1 to 4, Second Skin vs. classic film, what to avoid and when to actually worry.

A tattoo isn’t done when you leave the studio. The first four weeks decide how sharp, how clean and how lasting your piece stays. This guide is the same one we hand our clients in Zürich after their session — structured by day, no myths, no unnecessary drama.

If you had a fine line, lettering or mini tattoo session with us, healing is generally faster and less complicated than with traditional tattoos. Even so: each phase has its own rules.

The two routes: Second Skin vs. classic film

Right after the session you’ll get one of two protective options. Which one suits you depends on your piece, your daily life and your skin.

Second Skin (Saniderm, Dermalize, Tegaderm)

Thin, transparent, breathable film that stays on the skin for up to 5 to 7 days. It’s waterproof and protects the tattoo completely from friction, bacteria and loss of saturation.

  • Stays on for 3 to 7 days
  • You can shower (but not bathe)
  • Wound fluid collects under the film — this is normal
  • If the film loosens or air gets in, change or remove it

Classic film (cling wrap)

Worn only for the first 2 to 4 hours, then removed, washed, and continued with ointment. This route demands more discipline from you, but for very large pieces or sensitive skin it’s sometimes the better choice.

  • Film off after max. 4 hours
  • Gentle wash with lukewarm water
  • Thin layer of aftercare product two to three times a day
  • Tattoo shouldn’t dry out, but also shouldn’t suffocate

At our Zürich studio we work with Second Skin as default. It’s cleaner, simpler, and significantly reduces the risk of scabbing and colour loss. For adhesive allergies we switch to the classic route.

Day by day through the first weeks

Day 0 — the appointment itself

Right after tattooing the skin is red and slightly swollen. We seal it with Second Skin immediately. You go home and do: nothing special.

  • Avoid intense sweating for the next few hours
  • Sleep in loose clothing
  • Drink plenty of water
  • No alcohol, no sport

Days 1 to 3

An amber-coloured fluid (plasma, lymph and a little residual pigment) collects under the Second Skin. This is normal and expected. It does not mean your tattoo is “leaking out”.

  • Leave the film alone
  • Showering is fine (the film is waterproof)
  • No bath, no sauna, no swimming pool
  • Sleep loosely, ideally not lying directly on it

If the film starts to lift at an edge or air gets in, it’s time for a change. You can swap the film yourself or come into the studio — we do it for free.

Days 3 to 7 — film off

Between day 3 and day 7 you remove the Second Skin. Best done under a warm shower, slowly and in the direction of hair growth. Don’t rip it off.

  • Wash the tattoo gently with lukewarm water and fragrance-free soap
  • Pat dry, don’t rub
  • From now on, two to three times a day apply a thin layer of aftercare cream
  • Recommended: Bepanthen healing ointment, Hustle Butter, Tattoo Goo

In the next few days the skin will become slightly dry and a thin layer will flake off — like a mild sunburn. Don’t scratch, don’t peel. The pigment is already safe in the dermis; what comes off is just dead surface skin.

Days 7 to 14 — micro-peeling

The skin shows fine flakes, similar to after a sunburn. The tattoo can look slightly milky or faded during this phase. Don’t worry — once the upper skin layer is fully renewed, saturation comes back.

  • Continue creaming two to three times a day
  • Light to moderate sport okay, no direct pressure or friction on the area
  • Strict sun avoidance still
  • No swimming pool, no sea, no lake

Days 14 to 28 — the invisible healing

On the surface everything looks fine. Inside, the dermis is still regenerating. Pigment binding is most fragile in this phase — a sunburn now can permanently damage the tattoo.

  • Reduce creaming to once a day, then as needed
  • SPF 50 on the area whenever it’s exposed to sun
  • Swimming is fine again after day 21
  • Sauna after day 28

From week 5 your tattoo is officially fully healed. Even so: sun protection for life. Nothing fades a tattoo faster than UV.

What to avoid in the first 4 weeks

This list is shorter than you’d think — but every point counts.

  • Pool, lake, sea — until day 21 (bacteria and chlorine)
  • Sauna and steam room — until day 28 (sweat lifts scabs)
  • Direct sun — strictly first 4 weeks, then always with SPF
  • Tanning bed — none at all the first 4 weeks
  • Intense sport with friction — first 7 days
  • Tight clothing over the area — first 7 days
  • Scratching or picking at flakes — completely off limits
  • Perfume, alcohol-based body lotion on the area — first 14 days
  • Pet licking — until fully healed

The right aftercare cream — what actually works

There are countless tattoo aftercare products. The honest truth: you don’t strictly need any of them. What works:

  • Bepanthen healing ointment (pharmacy, 5 to 10 CHF) — classic, has worked for decades
  • Hustle Butter (tattoo shops, around 25 CHF) — vegan, smells nice, very economical
  • Tattoo Goo — purpose-made for tattoos, mid-price
  • Pure organic shea butter — cheap, very gentle, perfect for sensitive skin

What you should not use: Vaseline (clogs the skin), perfumed body lotion, coconut oil in the first days (too rich).

When to actually worry

99 percent of all healing journeys are uncomplicated. With these signs though, contact us or a doctor:

  • Strong redness spreading outward from the tattoo
  • Pus-like, yellowish discharge (clear plasma is normal)
  • Fever or chills
  • Pain that gets worse after day 3 instead of better
  • Blister formation that doesn’t subside within 48 hours
  • Allergic reaction to the Second Skin (itching with red welts at the film edge)

In a normal healing, the tattoo isn’t painful after day 3, at most slightly tight. If it gets worse instead of better, that’s a warning sign.

Touch-up after 4 weeks — when is it needed?

Most fine line tattoos heal perfectly without a touch-up. If a line did get skipped or an area is paler than wanted, we offer a free touch-up appointment within 6 months. Earliest 4 weeks after the first session — before that the skin isn’t stable enough.
